The M Health Fairview Center for Community Health Equit y is advancing community health equity through partnerships, research, leadership development and initiatives focused on addressing the social and structural factors that influence health outcomes. As part of this work, M Health Fairview has announced the Center for Community Health Equity Accelerator Fellowship , a year-long leadership development programme designed to support community leaders working on innovative projects that promote health equity and address the social determinants of health. The Center works alongside community partners toward a shared goal of improving the health of the communities it serves. It is located within the Fairview Community Health and Wellness Hub and provides a space for M Health Fairview and community partners to collaborate on reducing racial and other disparities in community health outcomes. About the M Health Fairview Center for Community Health Equity The Center for Community Health Equity extends the work of M Health Fairview’s Community Advancement team. Its approach brings together the health system, community organizations, leaders and residents to address factors that influence health beyond traditional clinical care. The Center’s work is built around three key strategies: 1. Community Incubator This strategy focuses on addressing the social determinants and drivers of health as well as individual social risk factors. 2. Engagement Infrastructure The Center works to build trusting partnerships while creating opportunities for community and patient voices to inform its work. 3. Inclusive Institution The Center also works toward transforming structures to create an increasingly equitable and inclusive environment .
